ALL PLAYERS WILL BE PLACED IN & PLAY FOR THEIR CORRECT AGE GROUP .

By completing this form:

a. The parent (or guardian) agrees to their son/daughter receiving emergency medical treatment (which may include medication, anaesthetic or blood transfusion) as considered necessary by the medical authorities present.
b. The parent (or guardian) son /daughters/players agree to abide by KPR, FA, SCFA and the relevant Leagues codes of conduct. Help us keep football safe and fun for kids.
b. Unless the club is advised in writing (e-mail), the parent (or guardian) agrees to photos of their son/daughter being published, un-named, on the club website.
c. Players will not be Eligible to play or train for KPR and KPR will keep their registration card until fees have been paid.
